Clean up schema of the "merge_requests" table

This adds various foreign keys and indexes to the "merge_requests" table as outlined in https://gitlab.com/gitlab-org/gitlab-ce/issues/31825. Fixes https://gitlab.com/gitlab-org/gitlab-ce/issues/31825
